Tools: Facility Improvements

Sometimes healthcare practitioners are not able to apply their new knowledge because of systems issues. For example, a highly trained nurse may not be able to facilitate patient care because she is too busy doing clerical work. Or, a minimal number of patients might be seen in a day because there is no triage system. Highly skilled clinical mentors can also address these issues. The instruments below can be completed by a clinical mentor during his or her site visit, along with a description of the types of work or guidance the mentor provided. Over time, the data collected by these instruments can indicate whether the clinical systems are improving over time. Ideally, improvements in clinical systems and improvements in provider practice would be mutually reinforcing.

    This tool was designed to capture changes in clinical systems over time. Clinical mentors were asked to complete the forms based on their own observations or quick conversations with facility staff. The objective was to get good enough information for programming decisions without placing an undue burden on facility staff or the clinical mentor.

    This tool was developed for use in Namibia to identify obstacles preventing transfer of learning from PMTCT and VCT training. The tool is used in the context of a comprehensive site visit conducted by in-service tutors that included healthcare worker observation, client exit interviews, and interviews with supervisors/managers. The document includes checklists for assessment of PMTCT services in an antenatal setting and in a labour/delivery ward.
